  • Our office is open Monday - Friday 8:30am - 5:30pm

Basic job duties:

  1. Accept clients for service on the basis of a reasonable expectation that the client's medical, nursing and social needs are met adequately by the agency in the client's place of residence
  2. Perform the initial assessment (OASIS) to determine the immediate care and support needs of the client 
  3. Review client's record and obtain medical history and other pertinent information 
  4. Meet with the client or client's family or guardian to discuss services and admission criteria
  5. Contact client's physician as applicable to obtain orders and verify diagnosis if the client or client's representative agrees to services.
  6. Refer to appropriate clinical services for assessment and initiate admission process

Required Knowledge, Skills and Abilities:

Must possess the ability to make independent decisions, follow instructions, and accept constructive criticism. Must be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must be able to work with ill, disabled, elderly, and emotionally upset people within the facility. Must be able to speak, write and understand English in a manner that is sufficient for effective communication with supervisors, employees, residents, and families.


Required Licensure:

Must possess a current and active license to practice as an RN in the State of Minnesota.
